Job summary

Valley Health seeks a Home Health Physical Therapist to deliver direct patient care in the patient’s home as part of a multidisciplinary team. The role includes clinical evaluation, plan of care development, and collaboration with nursing, OT, speech, social work, and home health techs.

Upon hire, you will undergo a 4–6 week orientation with a seasoned preceptor to ensure readiness. A valid PT license in VA or WV is required.

Qualifications

  • Bachelor's, master's or doctoral degree in Physical Therapy is required.
  • BLS Certification (American Heart Association) required prior to orientation.
  • Two years of clinical experience (4,160 hours) required.
  • Physical Therapy license or eligibility in Virginia or West Virginia.
  • Must have valid driver's license and transportation.
  • Basic computer/keyboard skills required; ability to transfer electronic data.

Responsibilities

  • Provide direct patient care in the patient’s home as part of a multidisciplinary team.
  • Supervise care provided by assistants.
  • Design individualized plan of care using professional judgment.
  • Complete documentation in electronic records on a laptop.
  • Participate in orientation and perform performance improvement activities.

Job description

New hires may be eligible for relocation assistance. They may also be eligible for a sign-on bonus of $15k or $25k dependent on experience, OR up to $30k in student loan reimbursement.

The Home Health Physical Therapist provides direct patient care in the patient's home environment as part of a multidisciplinary team including nursing, physical therapy, occupational therapy, speech, social work and home health techs. The Physical Therapist evaluates patients and use professional judgment to design the individualized plan of care which best meets the patient's needs. The Physical Therapist provides direct, one-on-one patient care, and will also supervise care provided by assistants. The Home Health agency relies heavily on technology; email is a primary means of communication, and the therapist will use a laptop to complete documentation in an electronic record. The patient population varies widely across the spectrums of acuity, age, and diagnosis. The Physical Therapist works under the direction of a clinical team manager with support from a rehab clinical coordinator. Employment begins with a 4–6-week orientation with a seasoned preceptor to ensure that the clinician is well prepared when they begin treating on their own. Additional job duties include performance improvement opportunities, policy and procedure review and development, research, etc. Working in home health provides endless variety of setting, scenery, and clinical challenges, and provides a unique opportunity to enter and work within the real-life environments of our patients, bringing meaningful change to their daily lives.

Education

Bachelors, master's or doctoral degree in Physical Therapy is required.

A completed CI credentialing course, home study adult learning course, or equivalent, is required.

Experience

Minimum two years of clinical experience (4,160 hours) is required.

Certification & Licensure

Physical Therapy licensed or eligible in the state of assignment (Virginia or West Virginia) is required. Licensure in a secondary state may be required depending on geographical area assigned.

BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved is required. New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.

Valid driver's license and reliable transportation is required.

Qualifications

Basic computer and keyboarding skills or participation in computer training within the first year of employment.

Competent to successfully interact with patients, according to their special needs which may apply based upon the age of the patient served.

Must have access to transfer electronic data.

Must not have been convicted of or have pending charges of any barrier crimes.
