Job summary

Amedisys, Inc. is looking for a part-time Physical Therapist to support patients in their homes across New Hampshire. You will evaluate mobility, design and deliver interventions, reexamine every 30 days, and adjust plans to restore function.

This role emphasizes home-based care and patient comfort. Responsibilities include discharge planning and collaborating with care teams. Eligible candidates will hold a PT license in the state, CAPTE education, CPR, and some home health experience;

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• One year of related PT experience for home health; mentoring required if less than one year.
• CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days at minimum, modify interventions as needed, and perform discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you're assigned to work in.
• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
