Qualifications

  • 1 year clinical experience in a healthcare setting or active enrollment in the Providence postgraduate preceptor program.

Physical Therapist- Providence Home Services Oregon

3,000 hiring bonus for eligible external hires that meet required qualifications and conditions for payment .

Schedule : Full time, Days, Monday- Friday (8 : 00am - 4 : 30pm)

Responsible for East Portland area

Manages and provides skilled physical therapy to patients / clients with a wide variety of diagnoses and disabilities in a home setting in accordance with a physician plan of treatment, taking into account individual patient special or age-related needs. Requires skills in patient / client evaluation and treatment, as well as professional consultations to achieve maximum patient / client independence in accordance with the individual's functional level and potential.

Required Qualifications :

  • Bachelor's Degree from an accredited Physical Therapy Program. or
  • Master's Degree from an accredited Physical Therapy Program. or
  • D.P.T. from an accredited Physical Therapy Program.
  • Within 30 days of hire : National Provider BLS - American Heart Association
  • Upon hire : Oregon Physical Therapist License
  • Upon hire : Washington Physical Therapist License - if practicing within this state
  • 1 year Clinical experience in Home Health or healthcare setting, or active enrollment in the Providence postgraduate preceptor program.

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses.
