Claremont Care Center in Pomona, CA is seeking a Full Time Physical Therapist to join our team. You will provide rehabilitative services, contribute to wound care and neuro rehabilitation programs, and support patients’ recovery journeys.

The role offers a flexible schedule with work-life balance and pay ranging from $54 to $58 per hour based on experience. New grads are welcome to apply and licensure is required.

Qualifications

• Licensed Physical Therapist with current license.
• Open to therapists at all experience levels, including new grads.
• Ability to perform standing/walking and assist with patient transfers as needed.

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Perform procedures as part of the rehabilitation plan including exercises and wound care.
• Provide counseling to patients and families regarding physical disorders.
• Generate discharge summaries compliant with regulatory requirements.

Claremont Care Center is a beautiful skilled nursing facility nestled in the heart of Pomona, CA. Our facility is surrounded by a lush garden setting, and we are very proud of our updated décor, spacious and well-equipped therapy gym, and comfortable, well-maintained environment. We are seeking a Full Time Physical Therapist to join our team. As part of this amazing team of clinicians, you will have opportunities for training and continuing education in Think Thin/dysphagia, Abilities Care and dementia, stroke and neuro rehabilitation, and respiratory/COVID recovery programs.

Full Time: Flexible schedule with work-life balance

Pay: $54 - $58/hour based on experience

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Physical Therapist license is required.
• Open to Physical Therapists at all experience levels. New Grads are welcome!
