Key Responsibilities

• Provide physical therapy services across a mix of outpatient and inpatient settings
• Evaluate patients and develop individualized treatment plans to address functional deficits and rehabilitation goals
• Deliver therapeutic interventions and monitor patient progress throughout the course of care
• Collaborate with interdisciplinary team members to coordinate patient care and discharge planning
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of Montana or eligibility to obtain
• Graduate of an accredited Physical Therapy program
• Ability to work effectively in both outpatient and inpatient care environments

Preferred Experience / Skills

• Prior experience in a multi-setting or blended outpatient/inpatient environment
• Strong communication and interpersonal skills for working within a collaborative care team
• Experience with a diverse patient population across varying acuity levels
