Job summary

Covenant Home Health Agency seeks a Physical Therapist to deliver skilled therapy to residents across Arizona senior living communities. You will evaluate referrals, develop goals, implement plans, and help patients regain mobility and independence.

Ideal candidates hold an Arizona PT license, CPR certification, and home health experience. This PRN role covers West Valley and East Valley with supportive team and benefits.

Qualifications

• Valid Physical Therapist license in Arizona, in good standing.
• Two years of Home Health experience as a Physical Therapist preferred.
• Possesses and maintains current CPR certification.
• Licensed driver with reliable vehicle and current vehicle insurance.
• Experience in geriatric physical therapy or working with seniors is preferred.
• Ability to work independently and manage time efficiently.
• Excellent observation, verbal and written communication skills, problem solving skills, and organizational skills.
• Excellent communication and interpersonal skills to work effectively with residents, families, and the healthcare team.

Responsibilities

• Evaluate physician’s referral and patient’s medical record.
• Assess therapeutic/rehabilitative/functional status using approved evaluation techniques and observations.
• Evaluate the home environment.
• Determine priority needs for physical therapy.
• Direct physical therapy treatment.
• Report to physician, patient’s response to treatment and changes in condition.
• Identify patient and family/caregiver needs for other home health services and refer as necessary.
• May train patients in the use of prosthetic device.
• Administer prescribed PT program to restore and maintain maximum function and prevent disability.
• Maintain documentation in the patient chart and re-evaluate progress.
• Communicate PT philosophy and goals to patients, families and rehab team.
• Supervise Physical Therapy Assistants according to policy and state regulations.

Overview

We are seeking a Physical Therapist to provide skilled physical therapy services to patients across Senior Living Communities. In this role, you will be instrumental in helping seniors recover, enhance mobility, and maintain their independence through tailored physical therapy treatment plans.

Responsibilities
