Job Position:

Physical Therapist for an In-house Skilled Nursing Facility

Job Type:

PRN

Job Purpose:

To assist in planning, organizing, developing, and directing Physical Therapist services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by DOR, Administrator, or Regional support team to ensure the highest quality of resident care.

Job Functions:

1. Provide Physical Therapist services to residents according to established standards, procedures, and productivity standards.
2. Adhere to procedures regarding documentation and billing of services.
3. Evaluate and develop resident therapy programs with sound judgment.
4. Conduct regular screenings to assess the need for intervention/treatment.
5. Evaluate residents to gather data for treatment planning.
6. Develop treatment plans with clear goals and methods.
7. Communicate evaluation findings and treatment plans to residents, families, and team members.
8. Complete compliant and necessary documentation in NetHealth EMR systems.

Education & Requirements:

1. Bachelor of Science Degree in Physical Therapy from an accredited program.
2. Active Physical Therapy license in Pennsylvania.
3. Ongoing continuing education and professional development.
4. Experience in Long-term care or Skilled Nursing Facility preferred.
