Benedictine Living in Owatonna, MN is seeking a full-time Physical Therapist to evaluate residents, plan and implement therapy interventions, and contribute to discharge planning.
The role requires current state licensure in Physical Therapy in good standing and a Bachelor's degree. You will educate the health care team, residents, and families on the care plan and collaborate to meet resident goals.
The Physical Therapist is responsible and accountable for residents’ evaluations, ongoing assessments, and the planning and implementation of therapy interventions.
Develops and revises plan of care based on the residents’ needs and establishes functional goals, outcomes, and discharge plans. Plans for residents’ discharge and anticipates for long term and/or short term residents’ needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
