Job summary

Brookestone Home Health Care in Omaha is seeking a dedicated Physical Therapist (PT) for part-time or PRN work in the home health setting. You will evaluate, treat, and customize plans to improve mobility, balance, and independence for patients in the Omaha area and surrounding communities.

The position offers flexible scheduling with a patient-centered approach, collaboration with an interdisciplinary team, and opportunities to impact patients’ quality of life at home.

Qualifications

• Current Nebraska Physical Therapist license in good standing.
• Current CPR certification.
• Valid driver's license, reliable transportation, and proof of automobile insurance.
• Ability to travel within the service area.

Responsibilities

• Perform comprehensive physical therapy evaluations and reassessments.
• Design and implement individualized treatment programs.
• Provide therapeutic exercises, gait and balance training, transfer training, and fall prevention interventions.
• Assess and recommend assistive devices and durable medical equipment.
• Educate patients and caregivers on treatment strategies and home exercise programs.
• Track progress and adjust treatment plans to achieve optimal outcomes.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate care.
• Complete accurate and timely clinical documentation.
• Adhere to agency policies, professional standards, and regulatory requirements.

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our team on a Part-Time or PRN basis. Position Overview
