Job summary

Emerald Nursing & Rehab Mercy in Omaha, NE is seeking a skilled Physical Therapist (PRN) to assess, plan, and implement individualized therapy programs for residents in a skilled nursing facility.

You will work closely with a multidisciplinary team to promote functional independence, deliver evidence-based interventions, educate patients and families, and document progress in accordance with facility policies and regulatory requirements.

Qualifications

• Doctoral or Master’s degree in Physical Therapy from an accredited program.
• Current state licensure as a Physical Therapist.
• Experience in skilled nursing or long-term care preferred.
• Compassionate, patient-focused and detail-oriented.

Responsibilities

• Patient Assessment: Evaluate patients’ physical conditions, mobility, and functional abilities to develop comprehensive treatment plans.
• Treatment Planning: Create and implement personalized therapy programs that address goals and improve mobility.
• Therapeutic Interventions: Administer therapeutic exercises, manual therapy, and modalities to support rehabilitation.
• Progress Monitoring: Regularly assess and document patient progress and adjust plans as needed.
• Patient Education: Educate patients and families on exercises, safety, and home care strategies.
• Collaboration: Work with physicians, nurses, and other health professionals to ensure coordinated care.
• Documentation: Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries.

Qualifications
