Angels Care Home Health is seeking a Physical Therapist to plan, implement, and evaluate patient care plans to restore or maintain well-being. You will organize PT programs based on referrals and collaborate with an interdisciplinary team to optimize outcomes.

The role requires a state PT license, 2+ years of experience, CPR certification, and knowledge of DME. We offer a Monday–Friday schedule, full benefits, and opportunities for growth.

Qualifications

• Licensed Physical Therapist in the state.
• Minimum 2 years of PT experience.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment (DME).

About the Position:

Plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation. Collaborates with interdisciplinary team.

Qualifications:

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.
