Angels Care Home Health is seeking a licensed Physical Therapist to plan, implement, and evaluate patient care plans for home health patients in Oklahoma. You will conduct PT treatment programs based on medical referrals and collaborate with an interdisciplinary team to optimize patient outcomes.

Qualifications

• Active state license as a Physical Therapist in Oklahoma.
• Minimum 2 years of PT experience.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

About the Position:

Plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Plans, organizes, and conducts physical therapy treatment programs based on the medical referral and their evaluation. Collaborates with interdisciplinary team.

Qualifications:

• Current state license as a Physical Therapist.
• Two years of experience as a Physical Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.
