Role Description

The role also involves collaborating with other healthcare professionals, maintaining a safe and welcoming treatment environment, and supporting continuity of care, including coordination of home care as needed.

Qualifications

• Strong foundation in Medicine and Physical Therapy, with the ability to apply clinical knowledge to diverse patient conditions.
• Demonstrated skills in Rehabilitation, including treatment planning, therapeutic exercise, and progression of care.
• Proficiency in Patient Care, emphasizing safety, empathy, communication, and cultural sensitivity.
• Experience or interest in Home Care, including educating patients and families on exercises and self-management strategies.
• Licensed or license-eligible Physical Therapist in the state of New York.
