Pomera Care LLC in Bronx, NY is seeking a Physical Therapist to deliver high-quality rehabilitative care to diverse adult and geriatric populations. You will implement personalized care plans to promote recovery, mobility, and overall well‑being, using hands‑on techniques and patient education.

This full‑time role offers $55–$60 per hour, with health, vision and dental insurance, life and disability insurance, and PTO.

Qualifications

• Licensed Physical Therapist.
• 1+ years of PT experience in outpatient, inpatient, or rehab.
• Fluency in Spanish preferred.

Responsabilités

• Develop and implement individualized treatment plans for adult and geriatric patients, focusing on functional improvement, mobility, and pain reduction.
• Deliver manual therapy interventions, including trigger point techniques, soft‑tissue mobilization, and joint mobilization as clinically appropriate.
• Monitor patient progress, ensure safety during all therapeutic activities, and provide ongoing motivation throughout the rehabilitation process.
• Educate patients and families on therapeutic exercises, home‑based recovery strategies, and long‑term wellness practices.
• Document evaluations, treatment sessions, and progress notes accurately within eClinicalWorks in compliance with regulatory and clinical standards.
• Collaborate with interdisciplinary teams to support coordinated, patient‑centered care and optimize clinical outcomes.
