Job summary

Better Healthcare OT/PT, PLLC in Brooklyn, NY is seeking a Physical Therapist for a full-time position in a skilled nursing facility. You will evaluate referred patients, develop and implement treatment plans, and collaborate with physicians and case managers.

You may supervise PT aides and assistants, document care, and ensure HIPAA compliance while delivering compassionate, evidence-based therapy to residents.

Qualifications

• Graduation from an accredited physical therapy program.
• Active NYS PT license in good standing.

Responsibilities

• Meets the patient's goals and needs and provides quality care by assessing and interpreting evaluations, determining physical therapy treatment plans in consultation with physicians or by prescription.
• Undertake a comprehensive examination/assessment/evaluation of the patient/client needs.
• Evaluates progress and adjusts treatment as needed.
• Documents patient care in charts and department records.
• Maintains patient confidentiality and protects facility operations by following HIPAA and related policies.
• Supervise Physical Therapy Assistants and cosign PT Assistant progress notes.
• Protects patients and staff by adhering to infection-control policies and protocols.
• Complies with federal, state, and local regulatory requirements.”

JOB SUMMARY

The Physical Therapist is responsible for evaluating/assessing needs of referred patients and formulating treatment plans in a skilled nursing facility setting. Provides therapy services defined in treatment plans. Work cooperatively with physicians, and case managers. May supervise physical therapy assistants, and aides. Familiar with standard concepts, practices, and procedures within a particular field. Relies on experience, and direction from manager to plan and accomplish goals. Performs a variety of tasks.

QUALIFICATIONS

• Graduation from an accredited physical therapy program
• NYS PT License Certification by New York State Office of the Professions and practices Physical Therapy according to the state requirements - active NYS License in good standing.

JOB RESPONSIBILITIES

• Meets the patient's goals and needs and provides quality care by assessing and interpreting evaluations, determining physical therapy treatment plans in consultation with physicians or by prescription.

• Maintains patient confidence and protects Nursing Home /Care Center operations by keeping protected health information confidential.
• Supervise Physical Therapy Assistants and cosigns Physical Therapy Assistant's progress notes.
• Protects patients and employees by adhering to infection-control policies and protocols.
• Complies with federal, state, and local legal and certification requirements by studying existing and new legislation.
