• Complete required forms and documentation per company policy and regulations

Responsibilities:

• Evaluate residents and patients with temporary or long-term disabilities and develop individualized treatment plans

• Administer physical therapy treatments to relieve pain, restore or improve function, and promote healing

• Assure all treatment is delivered in accordance with an established plan of care

• Provide clinical support and educate patients, families, and caregivers

• Monitor patient response to treatment interventions and adjust treatment plans as appropriate

• Complete required forms and documentation in accordance with company policy and state/federal regulations

• Collaborate with the interdisciplinary team to provide quality, resident-centered care
