Physical Therapists (PT) will provide one-on-one therapeutic treatment and care to patients to promote their health and independence using a comprehensive and patient-specific care plan.

**PT License in New York is required**

Placement offered in the following setting: HOME CARE AND SKILLED NURSING AVAILABLE.

**W2 and 1099 options available! Rate is dependent on setting, experience, and employment status with benefits for those interested in W2 full time**

Physical Therapist, PT Home Care tasks will include:

• Evaluates and treats patients recovering from injury, disease, or surgery in a home care environment.
• Establishes treatment plans and treats patients using a variety of methods, including exercise, stretching maneuvers, hands-on therapy, and equipment.
• Develops and carries out individualized treatment plans to help patients attain therapeutic goals.
• Tracks and documents treatments, responses, and progress; reevaluates and redefines therapeutic goals as necessary.
• Recommends and fits patients for assistive devices for self-care and ambulation if necessary.
• Provides training for patients, caregivers, and families on using assistive modalities and appropriate physical therapy procedures.
• Maintains professional and technical knowledge by attending educational workshops and participating in professional societies.
