Job summary

Millennium Physician Group is seeking a licensed Physical Therapist to join its Home Health Team serving Collier County, Florida. You will assess patients in their homes, create individualized treatment plans, and provide interventions to improve mobility, strength, and functional independence.

The role emphasizes collaboration with an interdisciplinary team, patient and family education, and strict documentation to ensure regulatory compliance and high-quality care.

Qualifications

• Current and valid Physical Therapist license in Florida.
• Minimum of 1 year of clinical experience as a Physical Therapist, preferably in home health.
• Strong clinical, communication, and interpersonal skills.

Responsibilities

• Conduct comprehensive evaluations of patients' physical abilities and develop individualized treatment plans.
• Implement therapeutic techniques including manual therapy, therapeutic exercises, and gait training.
• Educate patients and families on body mechanics, home exercise programs, and injury prevention.
• Collaborate with physicians, nurses, and other healthcare professionals to develop care plans.
• Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries.
• Adhere to HIPAA and Medicare regulations.

You will collaborate with an interdisciplinary care team, educate patients and families on rehabilitation and injury prevention strategies, maintain accurate clinical documentation, and ensure compliance with all regulatory standards to deliver high-quality patient care and achieve optimal outcomes.

What You’ll Do

• Patient Assessment: Conduct comprehensive evaluations of patients' physical abilities, limitations, and functional needs to accurately diagnose impairments and develop individualized treatment plans.
• Therapeutic Interventions: Implement a variety of therapeutic techniques, including manual therapy, therapeutic exercises, and gait training to improve patients' mobility, strength, balance, and pain management.
• Patient Education: Educate patients and their families on proper body mechanics, home exercise programs, and injury prevention strategies to promote long‑term health and well‑being.
• Collaboration: Work collaboratively with physicians, nurses, occupational therapists, and other healthcare professionals to develop comprehensive care plans and ensure optimal patient outcomes.
• Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries.
• Compliance: Adhere to all relevant regulatory standards, including HIPAA and Medicare regulations.

What We’re Looking For

• Licensure: Current and valid Physical Therapist license in the state of Florida.

Compensation Range:

$81,492.00
to
$122,238.00

The anticipated base salary range represents the Company’s good‑faith estimate of the compensation it reasonably expects to pay for this position at the time of posting. Actual compensation will be determined based on factors including experience, skills, qualifications, geographic location, internal equity, and business needs.
