Job summary

Flagship Therapy in Mount Pleasant, SC seeks a Full Time Physical Therapist for its in-house rehab program at Oak Harbor. You will screen, evaluate and treat patients with physical functioning disorders and wounds, following regulatory and clinical standards.

• Screen and evaluate patients with physical disorders and/or wounds; develop care plans adhering to standards.
• Deliver rehabilitative, skilled, and medically necessary treatments per clinical requirements.
• Assist with standing, walking, lifting, transferring, and repositioning of patients and equipment.
• Administer procedures including manual techniques, ambulation, exercises, modalities, devices, and wound care.
• Provide counseling to patients, families, and caregivers regarding physical disorders.
• Generate discharge summaries ensuring regulatory and clinical compliance.

Oak Harbor in Mount Pleasant, SC is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Physical Therapist license is required.
• Open to PTs at all experience levels. New Grads are welcome!
