Flagship Therapy seeks a dedicated Physical Therapist for its Desert Blossom Post Acute site in Mesa, AZ to join an in-house rehab program serving geriatric patients. The role covers comprehensive screening, evaluation, and treatment within a post-acute setting.

We offer a competitive hourly pay range of $42-$52 based on experience, plus a stable full-time schedule and growth opportunities. Open to new grads; active PT license required.

Responsibilities

• Screen and evaluate patients with physical function disorders and wound care; craft care plans.
• Deliver rehabilitative and medically necessary treatment per regulatory standards.
• Perform rehabilitation procedures including manual therapy, ambulation, exercises, modalities, and wound care.
• Provide counseling to patients, families, and caregivers on physical disorders.
• Create discharge summaries ensuring regulatory and clinical compliance.

DESERT BLOSSOM POST ACUTE - MESA, AZ

Desert Blossom Post Acute in Mesa, AZ, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Full Time: Flexible schedule with work-life balance

Pay: $42- $52/hour based on experience

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

Physical Therapist license is required.
