Horizon Post Acute Care in Glendale, AZ, is seeking a full-time Physical Therapist to join our in-house rehab program. The role offers a competitive pay rate of $45-$50 per hour based on experience and a sign-on bonus.

Responsibilities

• Screen and evaluate patients with physical functioning disorders; develop care plans.
• Deliver rehabilitative, skilled treatments per regulatory standards.
• Administer procedures including manual therapy, ambulation, therapeutic exercises, modalities, and wound care.
• Provide counseling to patients, families, and other providers.
• Prepare comprehensive discharge summaries in compliance with requirements.

Horizon Post Acute Care in Glendale, AZ, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a Full Time Physical Therapist for our in-house rehab program!

Full Time: Flexible schedule with work-life balance

Pay: $45 - $50/hour based on experience

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

Physical Therapist license is required.
