Job summary

Tennessee Quality Care is hiring a Physical Therapist (PT) for full-time home health visits in Shelby County, TN. You will evaluate patients, develop treatment plans, and provide skilled PT services following physician orders and state guidelines.

The role emphasizes mobility and function improvements, patient and family education, and collaboration with the care team. A valid PT license and driver’s license are required; home health experience is preferred.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the APTA.
• Current PT license in the state of practice.
• Home Health experience is preferred but not required.
• At least two years of experience in an acute care or similar clinical setting preferred.
• CPR and First Aid certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients and develop individualized treatment plans based on physician orders.
• Provide skilled physical therapy services according to the plan of care and state guidelines.
• Help patients improve mobility, strength, balance and overall function.
• Educate patients and families on treatment plans and home exercise programs.
• Recommend adaptive equipment and home modifications to promote independence and prevent injury.
• Collaborate with physicians and the care team to ensure optimal outcomes.
• Complete clinical documentation and care plan updates in a timely manner.
• Supervise PTAs in accordance with policy and state regulations.
• Encourage and motivate patients throughout their recovery journey.

What You’ll Do

• Help patients improve mobility, strength, balance, and overall function.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques.
• Recommend adaptive equipment and home modifications to promote independence and prevent injury.
• Collaborate with physicians and the interdisciplinary care team to ensure optimal patient outcomes.
• Accurately complete clinical documentation, progress notes, and care plan updates in a timely manner.
• Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations.
• Encourage and motivate patients throughout their recovery journey.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
• Current Physical Therapist (PT) license in the state of practice.
• Home Health experience is preferred but not required.
• At least two (2) years of experience in an acute care or similar clinical setting preferred.
• Current CPR and First Aid certification.
• Valid driver’s license, reliable transportation, and current automobile insurance.
