Job summary

Tennessee Quality Care is seeking a full-time Physical Therapist (PT) for Home Health in Shelby County, TN. You will evaluate patients, create individualized plans, and deliver skilled PT services in patients' homes, following physician orders and state guidelines.

Ideal candidates have an APTA-accredited PT program, current TN PT license, and at least two years of relevant experience. Comprehensive benefits, PTO, and mileage reimbursement are offered.

• Evaluate patients and develop individualized treatment plans based on physician orders.
• Provide skilled physical therapy services in accordance with the physician’s plan of care and state practice guidelines.
• Help patients improve mobility, strength, balance, and overall function.
• Educate patients, families, and caregivers on treatment plans, home exercise programs, and safety techniques.
• Recommend adaptive equipment and home modifications to promote independence and prevent injury.
• Collaborate with physicians and the interdisciplinary care team to ensure optimal patient outcomes.
• Accurately complete clinical documentation, progress notes, and care plan updates in a timely manner.
• Supervise Physical Therapist Assistants (PTAs) in accordance with company policy and state regulations.
• Encourage and motivate patients throughout their recovery journey.

Qualifications

• Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).


• Current Physical Therapist (PT) license in the state of practice.


• Home Health experience is preferred but not required.


• At least two (2) years of experience in an acute care or similar clinical setting preferred.


• Current CPR and First Aid certification.


• Valid driver’s license, reliable transportation, and current automobile insurance.
