Job summary

Medford Rehabilitation & Nursing Center seeks a licensed Physical Therapist to evaluate residents, plan treatments, and supervise staff. You will screen rehab needs, perform physician-ordered evaluations, and deliver therapy using modalities.

The role requires an MA license, graduation from an accredited PT program (or international equivalent), and ideally geriatric experience; driving and computer proficiency are preferred. We offer competitive compensation and comprehensive benefits.

Qualifications

• Current unencumbered MA license as a Registered Professional Physical Therapist.
• Graduate of an accredited school of physical therapy or international equivalent.
• Ability to drive and have dependable transportation.
• Able to use necessary computer applications.

Responsibilities

• Screen residents for rehabilitation therapy needs.
• Complete Physical Therapy evaluations upon physician order.
• Provide Physical Therapy plan of treatments and modalities.
• Modify treatment program level, intensity and techniques based on resident response.
• Document resident progress toward PT goals.
• Discharge residents from Physical Therapy services when appropriate.
• Provide patient, staff and family education and in‑servicing.
• Supervise Physical Therapy Assistants and unlicensed assistive personnel.

Qualifications/Experience:

• Must have a current unencumbered MA license as a Registered Professional Physical Therapist, Bachelor’s degree or higher.
• Graduate of an accredited school of physical therapy or international equivalent
• Geriatric experience preferred
• A current unrestricted driver’s license and dependable transportation
• Able to use necessary computer applications

Essential Job Functions:

• Screens residents for rehabilitation therapy needs
• Completes Physical Therapy evaluations upon physician order
• Provides Physical Therapy plan of treatments and modalities
• Modifies treatment program level, intensity and techniques based on the resident’s response to specific treatment plan
• Documents resident progress toward Physical Therapy goals
• Discharges residents from Physical Therapy services when appropriate
• Makes appropriate recommendations for nursing regarding mobility, positioning and ROM resident needs
• Completes Daily Activity Schedule, progress notes and re-certifications per facility policy.
• Supervises Physical Therapy Assistants and unlicensed assistive personnel
• Provides resident, staff and family education and in-servicing
• Demonstrates working knowledge of Medicare Part A PPS case management
• Complies with Federal and State regulations related to rehabilitation therapy
• Completes all documentation to comply with professional standard and regulatory requirements
• Completes care plans and MDS as directed, per facility policy
• Participates in the discharge planning process with the interdisciplinary team
• Participates in facility meetings and committees as assigned
• Submits required reports and documentation in accordance with established acceptable time frames
• Possess a strong knowledge of Medicare, Medicaid and other payment sources and their responsibility in the SNF setting
• Maintains confidentiality of facility and resident information
• Respects all residents all resident rights
• Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately
