Job summary

Medford Rehabilitation & Nursing Center in Massachusetts seeks a qualified Physical Therapist to oversee rehab services for residents.

You will screen residents, perform PT evaluations, develop treatment plans, supervise staff, document progress, and coordinate with the interdisciplinary team.

Licensure in MA is required; bachelor's degree or higher; geriatric experience preferred; valid driver's license and computer skills are expected.

Qualifications

  • Must hold MA license as Registered Physical Therapist.
  • Bachelor’s degree or higher in Physical Therapy.
  • Geriatric experience preferred.
  • Driver’s license and dependable transportation.
  • Able to use necessary computer applications.

Responsibilities

  • Screen residents for rehabilitation therapy needs.
  • Complete Physical Therapy evaluations upon physician order.
  • Provide Physical Therapy plan of treatments and modalities.
  • Modify treatment program level, intensity and techniques based on the resident’s response.
  • Document resident progress toward Physical Therapy goals.
  • Discharge residents from Physical Therapy services when appropriate.
  • Make recommendations for nursing regarding mobility, positioning and ROM needs.
  • Complete Daily Activity Schedule, progress notes and re-certifications per facility policy.
  • Supervise Physical Therapy Assistants and unlicensed assistive personnel.
  • Provide resident, staff and family education and in-servicing.
  • Demonstrate knowledge of Medicare Part A PPS case management.
  • Comply with Federal and State regulations related to rehabilitation therapy.
  • Complete all documentation to comply with professional standards and regulatory requirements.
  • Complete care plans and MDS as directed, per facility policy.
  • Participate in discharge planning with the interdisciplinary team.
  • Attend facility meetings and committees as assigned.
  • Submit required reports and documentation in established time frames.
  • Possess knowledge of Medicare, Medicaid and other payment sources in SNF settings.

  • Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately
