Job summary

Delhi Rehabilitation and Nursing Center seeks a licensed Physical Therapist to evaluate and plan rehabilitation care for residents in a SNF setting. You will lead therapy sessions, document progress, and adjust plans per physician orders, aiming to optimize mobility and independence.

Responsibilities include supervising rehab staff, coordinating with the interdisciplinary team, and ensuring regulatory compliance.

Qualifications

• Must possess a current unencumbered NYS license as a Registered Professional Physical Therapist.
• Bachelor’s degree or higher from an accredited school of physical therapy or international equivalent.
• Geriatric experience preferred.
• Current unrestricted driver’s license and reliable transportation.

Responsibilities

• Screens residents for rehabilitation therapy needs.
• Completes Physical Therapy evaluations upon physician order.
• Establishes plan of care for treatments per physician order.
• Provides therapy plan of treatments and modalities.
• Modifies treatment program based on resident response.
• Discharges residents from Physical Therapy services when appropriate.
• Makes recommendations for nursing regarding mobility, positioning and ROM needs.
• Completes daily activity schedule, progress notes and re-certifications.
• Supervises Physical Therapy Assistants and unlicensed personnel.
• Delivers education to residents, staff and families.
• Maintains Medicare Part A PPS case management knowledge.
• Complies with Federal and State rehab regulations.
• Completes documentation per professional standards and facility policy.
• Uses clinical software per procedures.
• Completes care plans and MDS as directed.
• Assists in ordering equipment and participates in discharge planning.

• Completes Daily Activity Schedule, progress notes and re-certifications per facility policy.
• Supervises Physical Therapy Assistants and unlicensed assistive personnel
• Provides resident, staff and family education and in-servicing
• Demonstrates working knowledge of Medicare Part A PPS case management
• Complies with Federal and State regulations related to rehabilitation therapy
• Completes all documentation to comply with professional standard and regulatory requirements
• Utilizes the clinical software applications per policies and procedures
• Completes care plans and MDS as directed, per facility policy
• Assists in ordering necessary equipment for the facility and residents
• Participates in the discharge planning process with the interdisciplinary team
• Participates in facility meetings and committees as assigned
• Submits required reports and documentation in accordance with established acceptable time frames
• Possess a strong knowledge of Medicare, Medicaid and other payment sources and their responsibility in the SNF setting
• Complies with all facility policies and procedures
• Maintains confidentiality of facility and resident information
• Respects all residents all resident rights
• Reports all instances of potential resident abuse, neglect, and misappropriation of resident property immediately
• Follows sanitation and infection control policies and procedures
• Recommends corrective action plan for all identified problems
• Maintains a positive working relationship between the rehabilitation department and other departments
