Job summary

Spring Meadows Health & Rehabilitation is seeking a compassionate Physical Therapist to assess referred patients and provide direct care in a collaborative healthcare setting. You will develop and implement rehabilitation plans while documenting in PCC EMR and maintaining high standards of patient safety.

Qualified candidates hold a valid PT license, CPR certification, and a degree from an accredited PT program.

Qualifications

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (PCC experience preferred but not required).
• CPR Certification.

Responsibilities

• Provide high-quality PT assessments and direct patient care.
• Collaborate with care teams to develop and implement rehabilitation plans.
• Maintain accurate EMR documentation and patient records.
