Job summary

Hill Valley Healthcare is seeking a compassionate Physical Therapist to provide high-quality patient care in a supportive team environment.

You will conduct PT assessments of referred patients, deliver direct patient care, and document in PCC EMR. A valid PT license in Virginia and CPR certification are required; experience with EMR is a plus. Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid Physical Therapy license for Virginia.
• CPR certification and active clinical skills.
• Experience with patient assessments and direct care.

Responsibilities

• Perform PT assessments of referred patients and provide direct patient care.
• Document clinical findings and progress in PCC EMR.
• Collaborate with team for patient-centered care and discharge planning.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
