Job summary

Heartland Home Health, part of Gentiva, is seeking a Full-Time Physical Therapist to join our home-based care team in Indiana. You will deliver skilled PT services, evaluate patients, and develop individualized treatment plans with the referring physician.

This role requires a Master’s degree in Physical Therapy, current IN license or eligibility, CPR, and strong communication. You’ll collaborate with nurses, other therapists, and social workers to help patients regain mobility and live safely

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Current PT license in the state of employment (or eligibility to obtain).
• CPR certification required.
• Strong communication, assessment, and problem-solving skills.
• Ability to work independently in a variety of home environments.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Deliver skilled physical therapy per individualized Plan of Treatment with physician input.
• Evaluate patients and develop rehabilitation goals to improve mobility and function.
• Adjust treatment plans to achieve optimal outcomes.
• Collaborate with the interdisciplinary home health team to provide coordinated care.
• Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
• Document visits, assessments, and progress notes accurately and in a timely manner.
• Support patients in maximizing their independence and improving quality of life at home.

Schedule: Monday-Friday, 8am-5pmSalary, Full Benefits, with 401k matchSign-On Bonus - $5000

What You’ll Do as a Home Health Physical Therapist:

• Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
• Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
• Adjust treatment plans and techniques as needed to achieve optimal outcomes.
• Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
• Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
• Document all visits, assessments, and progress notes accurately and in a timely manner.
• Support patients in maximizing their independence and improving quality of life—wherever they call home.

About You:

• 1+ year of experience in a hospital, home health, or outpatient rehab setting
• In-home therapy delivery
• Geriatric rehab, orthopedic or neurologic rehab
• Post-acute care or chronic condition management
• Working as part of a mobile or interdisciplinary care team
