Position Details
Schedule: Monday‑Friday, 8 a.m.–5 p.m.
Salary: competitive, full benefits, 401(k) match, $5,000 sign‑on bonus.
What You’ll Do
- Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed in coordination with the referring physician.
- Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
- Adjust treatment plans and techniques as needed to achieve optimal outcomes.
- Collaborate with the interdisciplinary home‑health team—including nurses, occupational therapists, speech therapists, and social workers—to provide coordinated care.
- Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
- Document all visits, assessments, and progress notes accurately and in a timely manner.
- Support patients in maximizing their independence and improving quality of life—wherever they call home.
Qualifications
- Master’s degree in Physical Therapy from an accredited program.
- Current PT license in the state of employment (or eligibility to obtain).
- CPR certification required.
- Strong communication, assessment, and problem‑solving skills.
- Ability to work independently in a variety of home environments.
- Valid driver’s license, auto insurance, and reliable transportation.
Preferred Experience (Not Required)
- 1+ year of experience in a hospital, home health, or outpatient rehab setting.
- In‑home therapy delivery.
- Geriatric, orthopedic, or neurologic rehab.
- Post‑acute care or chronic condition management.
- Working as part of a mobile or interdisciplinary care team.
