Job summary

Heartland Home Health, part of Gentiva, is seeking a Full Time Physical Therapist to join our Midland, MI team, covering the Saginaw territory. You will deliver skilled therapy at patients’ homes, collaborate with an interdisciplinary team, and educate clients on mobility and safety.

Requirements include a Master’s in Physical Therapy, Michigan PT license (or eligibility), CPR, and a valid driver’s license. A sign-on bonus is offered along with full benefits and a fleet vehicle program.

Qualifications

• Master's degree in Physical Therapy from an accredited program.
• Current PT license in the state of employment (or eligibility to obtain).
• CPR certification required.
• Strong communication, assessment, and problem-solving skills.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Deliver skilled physical therapy to patients based on an individualized Plan of Treatment developed with the referring physician.
• Evaluate patient needs and develop rehabilitation goals to improve mobility, strength, balance, and overall physical function.
• Adjust treatment plans and techniques as needed to achieve optimal outcomes.
• Collaborate with the interdisciplinary home health team—including nurses, occupational therapists, speech therapists, and social workers to provide coordinated care.
• Educate patients and caregivers on exercises, safety, mobility strategies, and use of assistive devices.
• Document all visits, assessments, and progress notes accurately and in a timely manner.
• Support patients in maximizing their independence and improving quality of life—wherever they call home.

About You
