Job summary

Engage Senior Therapy in Manchester, NH is seeking a dedicated Physical Therapist to join our team providing on-site therapy services at assisted living communities across New England. You will evaluate patients, develop personalized treatment plans, and deliver care with a high level of independence while collaborating with the Engage team.

A state license to practice PT is required or pending. We offer flexible scheduling, CEU reimbursement through MedBridge, and opportunities for leadership

Qualifications

• Graduation from an accredited physical therapy program.
• Active or pending state license to practice PT.
• Commitment to delivering quality, evidence-based patient care for seniors.

Responsibilities

• Evaluate patients to assess strength, balance, mobility, and functional impairments.
• Develop and implement personalized treatment plans to improve mobility and function.
• Deliver care at one or more assisted living locations, providing services to a diverse group of seniors.
• Provide care with independence while receiving guidance and resources from the Engage team.
• Educate patients and families on home exercise programs and strategies for independence.
• Document patient care in EMR in compliance with state and facility standards.
• Support professional growth through in-service meetings and workshops.

As a Physical Ther apist, you will:

• Evaluate patients to assess strength, balance, mobility, and functional impairments
• Develop and implement personalized treatment plans that focus on improving mobility, reducing pain, and enhancing overall function
• Deliver care at one or more assisted living locations , providing meaningful services to a diverse group of seniors
• Provide care with a high level of independence in assisted living communities, while always supported by the broader Engage team for guidance and resources
• Educate patients and families on home exercise programs and strategies for maintaining functional independence and a better quality of life
• Document patient care services in compliance with state and facility standards using EMR
• Enhance professional growth and development through participation in educational programs, in-service meetings, and workshops

About You

• Graduation from an accredited physical therapy program
• Active or pending state license to practice PT
