Lynnwood Post Acute Rehabilitation Center in Lynnwood, WA is seeking a PRN Physical Therapist to join our in-house rehab team. The role offers flexible PRN scheduling to support our geriatric and post-acute care programs.

You will conduct screenings, develop care plans, deliver skilled physical therapy, and provide patient and family counseling while adhering to regulatory standards. Open to therapists at all experience levels; new grads welcome; a physical therapy license is required to

Responsibilities

• Screen and evaluate patients with physical functioning disorders; develop care plans per standards.
• Deliver rehabilitation interventions per regulatory and clinical guidelines.
• Regular standing/walking; lifting, transferring and repositioning as needed.
• Administer procedures including manual therapy, ambulation, exercises, modalities, and wound care.
• Provide counseling to patients, families, and other providers.
• Generate comprehensive discharge summaries to meet regulatory requirements.

PHYSICAL THERAPIST - PRN

Lynwood Post Acute in Lynwood, WA, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a PRN Physical Therapist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Physical Therapist license is required.
