Early Intervention Physical Therapist

Responsible for coordinating treatment and implementing services for assigned clients, including outreach visits, as part of a multidisciplinary Early Childhood Services team.

Primary Functions:

• Provide outreach services to children and families
• Consult with team members to develop individualized plans and multidisciplinary services
• Maintain accurate client records, including IFSPs, assessments, progress notes, and contact logs
• Complete billing documentation as scheduled
• Follow professional standards and guidelines
• Keep supervisor informed of client progress
• Meet service requirements based on employment status
• Adhere to clinical recommendations and confidentiality standards
• Ensure compliance with billing and documentation protocols
• Conduct visits in natural environments based on client and family needs

Qualifications:

• Current licensure as a Physical Therapist by the Massachusetts Board of Allied Health Professions
