Job summary

Responsibilities include initial evaluations, progress tracking, documentation, patient and family education for home exercise programs, and staying current with PT advances.

Qualifications

• PT degree from an accredited program and valid state license.
• Minimum of 2 years of experience in a similar SNF setting preferred.
• Strong knowledge of anatomy, physiology, and kinesiology.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.

Position: Physical Therapist (P-Physical Therapist)

Salary Range: $40-43 per hour

Hours: 35 hours per week, Monday-Friday, with potential weekend shifts

Location: 86-bed Skilled Nursing Facility (SNF)

Key Responsibilities

• Monitor and document patient progress, adjusting treatment plans as needed
• Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive care for patients
• Educate patients and their families on exercises and techniques to continue their progress outside of therapy sessions
• Maintain accurate and up-to-date patient records and comply with all legal and ethical standards
• Keep current with advancements in the field of physical therapy and participate in continuing education opportunities
