Job summary

Creative Solutions Services, LLC seeks a Physical Therapist for a 13-week onsite contract in Lafayette, LA, starting 9/28/2026. You will evaluate patients, develop treatment plans, and monitor progress toward goals.

The position requires a Bachelor's in Physical Therapy, a valid state license, and a minimum of 2 years experience. Day shifts, 8-hour workdays, and collaboration within the Physical Therapy department are expected.

Responsibilities

  • Performs evaluations and provides treatment programs to facilitate patient specific goals.
  • Assesses the patient’s response to treatment and recommends treatment plan changes as appropriate.
  • Maintains patient records according to documentation standards, policies, and procedures.
  • Attains established productivity levels set forth by administration, including access to care, patient experience and quality.
  • Assists in program development and enhancement to meet patient and community needs.
  • Adapts behavior to the patient population and communication style.
  • Performs other related duties as assigned.

This job functions autonomously to deliver physical therapy treatment, either through referral or direct to consumer. Evaluates patients, develops treatment plans, assesses patients, and monitors progress toward treatment goals. Ensures patient needs are met through access to care, patient experience and quality of care metrics. Assists in program development and enhancement to meet patient and community needs.

Education Required

Bachelor s degree in physical therapy.

Experience Required

Minimum of 2 years

Certifications Required

Current physical therapy license in the state of practice.

  • Proficiency in using computers, software, and web-based applications.
  • Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals throughout the patient care process.
  • Excellent judgment and decision-making skills in order to deal with a variety of patients and diagnoses.

  • Ability to remain flexible with fluctuating schedules based on patient demand.
