Job summary

York Hospital in Maine seeks an experienced Physical Therapist for a full-time outpatient role serving our York, Wells, South Berwick and Kittery sites. The position is 36 hours per week, Monday through Friday with potential flexibility in office hours.

The therapist will evaluate patients, plan and implement care with physicians, initiate and revise the care plan, and provide high-quality restorative therapy across outpatient settings. State of Maine PT license and CPR required.

Qualifications

• Minimum of 1 year of physical therapy experience.
• State of Maine PT License.
• Basic Life Support (CPR) Certification.

Responsibilities

• Providing physical therapy services to patients in an outpatient setting in accordance with a written Plan of Care approved by a physician.
• The Physical Therapist makes an initial evaluation and plans with patient, and physician for on-going physical therapy services.
• Initiates, reviews and implements the PTCarePlan for each patient based on the assessment and prescribed Plan of Care.
• Revises Plan of Care when indicated.
