You will conduct patient evaluations, develop individualized plans, supervise staff, and collaborate with physicians to ensure high-quality care while maintaining detailed documentation and ongoing education for patients and caregivers.

Qualifications

• Graduate from CAPTE-accredited Physical Therapy program.
• Maine state PT license or eligibility to obtain one.
• Valid CPR certification.
• Strong knowledge of anatomy, physiology, and rehab principles.
• Excellent assessment and diagnostic skills.

Responsibilities

• Conduct thorough patient evaluations and develop individualized treatment plans.
• Implement evidence-based PT interventions and monitor progress.
• Document assessments, plans and progress notes with accuracy.
• Supervise PT assistants and rehab technicians.
• Establish measurable goals and adjust plans as needed.
• Educate patients and caregivers on treatment plans and home programs.
• Collaborate with referring physicians and other providers.
• Maintain compliant documentation and proper charting.

Job Description

We are seeking a dedicated Physical Therapist to join our healthcare team in Yarmouth, Maine. As a vital member of our rehabilitation services, you will provide comprehensive physical therapy to deliver optimal patient outcomes.

• Conduct thorough patient evaluations and develop individualized treatment plans
• Implement evidence-based physical therapy interventions and monitor patient progress
• Document patient assessments, treatment plans, and progress notes with attention to detail
• Supervise and coordinate care delivered by physical therapy assistants and rehabilitation technicians
• Establish measurable treatment goals and adjust plans based on patient progress
• Provide patient and caregiver education regarding treatment plans and home exercise programs
• Collaborate with referring physicians and other healthcare providers
• Maintain accurate and timely documentation in compliance with healthcare standards
• Utilize rehabilitation equipment and techniques effectively
• Participate in regular team meetings and care coordination discussions

Qualifications

• Graduate degree from a CAPTE-accredited Physical Therapy program
• Current Maine state physical therapy license or eligibility to obtain one
