Concordia-IRMC VNA is seeking a compassionate Physical Therapist to join our on-the-road team. You will perform comprehensive evaluations, craft individualized plans, and deliver evidence-based treatments to help patients regain function and independence.

You will educate patients and families, collaborate with a multidisciplinary team, and work in a flexible, autonomous setting that supports professional growth and excellent patient outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program with a valid state license.

What You'll Do as Physical Therapist

• Conduct comprehensive evaluations and develop individualized treatment plans that empower patients to reach their maximum potential.
• Implement evidence-based therapeutic interventions, including manual therapy, therapeutic exercises, and modalities.
• Monitor patient progress, adjust treatment plans as needed, and ensure timely and thorough documentation.

• Collaborate with physicians and other healthcare professionals to ensure a holistic approach to patient care.
