Job summary

VitalCaring Group is seeking a Home Health Physical Therapist to deliver patient-centered care in patients' homes, evaluating function, planning evidence-based treatment, and supporting families. You will collaborate with physicians and the care team to promote independence and safe transitions of care.

You will supervise PTAs as permitted by state guidelines, educate caregivers, and contribute to case conferences. Flexible scheduling and ongoing clinical education are offered.

Qualifications

• Graduate of an accredited Physical Therapy program (DPT or Master’s).
• Current PT license in the state of practice.
• Minimum 1–2 years of home health experience.
• Strong clinical assessment and critical thinking skills; excellent communication and patient education abilities.

Responsibilities

• Evaluate and assess patients’ functional status, mobility, strength, and rehabilitation potential.
• Develop and implement individualized care plans in collaboration with physicians and the care team.
• Provide hands‑on therapy to improve mobility, strength, balance, and independence.
• Educate patients and caregivers on treatment plans, exercises, and safety strategies.
• Monitor patient progress and adjust treatment plans as needed.
• Support safe transitions of care and contribute to discharge planning.
• Identify additional patient needs and collaborate with interdisciplinary teams.
• Partner closely with nurses, therapists, and physicians to ensure coordinated care.
• Document patient care and progress accurately and timely within the EMR.
• Participate in case conferences and team meetings.
• Supervise Physical Therapist Assistants (PTAs) in accordance with state guidelines.
• Provide guidance and education to caregivers and support staff.

What You’ll Do

What You Bring

• Current PT license in the state of practice

• Strong clinical assessment and critical thinking skillsExcellent communication and patient education abilities
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
