Job summary

Angels Care Home Health is seeking a licensed Physical Therapist to plan, implement, and evaluate patient care plans aimed at restoring or maintaining wellbeing. You will develop therapy programs based on medical referrals and patient evaluations and collaborate with an interdisciplinary team for coordinated care.

The ideal candidate holds an active PT license, has at least two years of experience, and maintains CPR and Hepatitis profiles.

Qualifications

• Active state license as a Physical Therapist.
• Minimum of 2 years of physical therapy experience.
• Current CPR and Hepatitis profile.

Responsibilities

• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
• Plans, organizes, and conducts physical therapy treatment programs based on referrals and evaluation.
• Collaborates with an interdisciplinary team to coordinate care.
