Flagship Therapy in Texas City, TX is hiring a Physical Therapist for a full-time role with PRN schedule options in our in-house rehab program. The Phoenix Post Acute - Texas City provides geriatric and post-acute care and a stable therapy team.

As a PT you will screen and evaluate patients, develop care plans, deliver skilled interventions, and document discharge summaries. Licensure is required; new grads are welcome and training is provided.

Qualifications

• PT license required.
• New grads welcome; all experience levels considered.
• On-site clinical role in post-acute care.

Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

The Phoenix Post Acute in Texas City, TX, is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated Full Time or PRN Physical Therapist for our in-house rehab program!
