Job summary

Amedisys is seeking a Per Diem Physical Therapist to help patients maintain, restore, and improve movement and function in home settings. You will deliver interventions, perform periodic reexaminations, and discharge planning as appropriate.

Qualifications include a state PT license, CAPTE-accredited education, CPR certification, and at least one year of home health experience or regional leadership approval for less experience.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from CAPTE-accredited physical therapy program.
• One year of related physical therapy experience for home health care; mentor approval if less than one year.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days at minimum), modify interventions as needed and perform discharge planning.
• Other duties as assigned.

Qualifications

• Current license to practice physical therapy specific to the state you’re assigned to work in.

• Graduate from an accredited physical therapy educational program as set by the Commission on Accreditation of Physical Therapy Education (CAPTE).
• One year related physical therapy experience for PTs entering home health care to ensure that knowledge and skills are sufficient to safely provide physical therapy services to patients.
• If less than one year of experience, approval from regional clinical leaders is required as well as appropriate mentoring and/or participating in a residency program.
• Current CPR certification.
