Optum Home Health is seeking a Physical Therapist to deliver one-on-one patient care in patients' homes across Indianapolis. This full-time or PRN role offers autonomy, a structured M-F schedule, and the opportunity to build meaningful patient relationships in a home-based setting.

You will assess mobility, create individualized plans, and work with an interdisciplinary team to support patient independence while documenting progress. Sign-on bonus and a per-visit pay model enhance compensation.

What you'll do

As a Home Health Physical Therapist, you'll evaluate patients in their homes, develop individualized plans of care and provide treatment focused on improving mobility, function and independence.

You'll collaborate with patients, families, physicians and the broader care team while managing your visits and documenting patient progress.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $34.23 to $61.15 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
