Optum is seeking a Home Health Physical Therapist in Indianapolis, IN to deliver one-on-one patient care in the home-based setting. This full-time or PRN role offers Monday through Friday scheduling, with autonomy and meaningful patient relationships in a supportive team.

You will evaluate patients, design plans of care, and provide home-based therapy to improve mobility and independence, while coordinating with families and the care team.

Formación

• Current Physical Therapy license in the state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, with access to dependable transportation.

As a Home Health Physical Therapist, you'll evaluate patients in their homes, develop individualized plans of care and provide treatment focused on improving mobility, function and independence.

You'll collaborate with patients, families, physicians and the broader care team while managing your visits and documenting patient progress.

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $34.23 to $61.15 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
