Job Purpose:

The Physical Therapist is responsible for directing and carrying out individualized programs of physical therapy, including assessment of impairment, developing and implementing plans of care, providing education to patients/families/caregivers, and communication to maximize patient progress. The Physical Therapist supervises Physical Therapy Assistants and Technicians.

Primary Duties and Responsibilities:
  1. Provides quality, efficient, patient-centered physical therapy treatment according to physician orders and plan of care.
  2. Makes patient care decisions under the Physical Therapy scope of practice.
  3. Develops and implements a plan of care considering the patient’s history, context, and goals.
  4. Effectively documents daily treatments, evaluations, and completion of progress notes to support each patient's individualized plan of care utilizing EMR and other electronic formats; communicates verbally and collaborates with the healthcare team.
  5. Ensures safety at discharge and continuation of progress by providing home exercise programs and recommendations for continued therapy at home.
  6. Maintains patient confidentiality and adheres to infection control and other safety policies.
  7. Attends and participates in organizational meetings for communication and process improvement.
  8. Interacts with a friendly demeanor with patients, caregivers, families, physicians, and all clinic staff.
  9. Communicates with fellow therapists, physicians, and administrative staff for scheduling and coordination of patient care.
  10. Contributes to the department's development by attending continuing education, mentoring new grads, and developing protocols.
  11. Participates in research projects and specialty-specific round tables.
  12. Aligns actions with the organization’s guiding platform, keeping patients at the center of all decisions.
  13. Supports and contributes to positive change in the organization.
  14. Communicates honestly and compassionately to build connections with patients and colleagues.
  15. Takes ownership of work, ensuring quality service to customers.

Education and Experience Requirements:
  1. Bachelor’s degree (B.A./B.S.) in Physical Therapy, or Master’s Degree or Doctorate in Physical Therapy required.
  2. Licensed in PT in the state of Illinois.
  3. Working knowledge of Microsoft and Google Suites.
  4. Familiarity with EMR and Physical Rehabilitation software.
Environmental/Working Conditions:

Working environment is in an office/clinic setting.

Physical/Mental Demands:
  1. Stand for long periods, walk, kneel, crawl, stoop, and climb stairs.
  2. Lifting, carrying, or moving heavier objects i.e., pushing or pulling up to 100 pounds and lifting and carrying up to 50 pounds.
  3. Full range of motion in shoulders and arms, including reaching above or below the shoulder.
  4. Finger dexterity, the ability to twist hands/wrists repetitively, and demonstrate exercises.
  5. Good vision and hearing to observe patient movements and verbalizations of pain.
  6. Operates modalities (e-stim, NMES, whirlpool), sharps (scalpels, tweezers, scissors).
