Job Summary
The Physical Therapist is responsible for supporting the treatment of illness and injury through physical exercises and therapies aimed at rehabilitation and restoration of body functions. This role involves providing individualized therapy programs to patients of all ages to promote maximum wellness and independence. The Physical Therapist develops and revises plans of care, applies therapeutic treatments, supervises assistants, and collaborates with healthcare teams to ensure high-quality patient care and compliance with regulatory standards.
Responsibilities
- Coordinate physical therapy services and provide direct therapeutic care to assigned clients.
- Teach rehabilitation skills to clients and caregivers to promote independent functionality and safety.
- Provide preventive and rehabilitative physical therapy procedures tailored to client age and needs.
- Develop and revise plans of care based on assessments, client history, diagnosis, physician orders, and goals.
- Perform or monitor treatments using physical agents such as heat, water, and electricity with appropriate equipment.
- Follow utilization guidelines and protocols for therapy treatments.
- Formulate and implement teaching plans; communicate clinical findings and changes to physicians and care team.
- Document all skilled and medically necessary care thoroughly, timely, and accurately in compliance with Medicare, State, Federal, JCAHO, insurance, and payer requirements.
- Maintain client and organizational confidentiality at all times.
- Participate in Quality Management programs and complete required in-service training to maintain skills.
- Supervise Physical Therapy Assistants and Home Health Aides as applicable, ensuring compliance with laws and policies.
- Conduct supervisory visits and prepare/update written instructions for Home Health Aides as needed.
- Participate in staff meetings, case conferences, business development, and customer service programs.
- Deliver high levels of customer service to internal and external customers.
- Demonstrate leadership competencies through communication, relationship building, and teamwork.
Required Skills & Certifications
- Licensed Physical Therapist in the state of practice.
- Experience in physical therapy treatment and rehabilitation.
- Knowledge of physical therapy practice guidelines and regulatory compliance.
- Strong documentation and communication skills.
- Ability to develop and implement individualized care plans.
- Competence in using physical therapy equipment such as whirlpool baths, electrical muscle stimulation, and ultrasound.
- Commitment to maintaining client confidentiality and quality standards.
Preferred Skills & Certifications
- Experience supervising Physical Therapy Assistants and Home Health Aides.
- Familiarity with Medicare, State, Federal, and JCAHO regulations.
- Participation in quality improvement and customer service initiatives.
- Leadership and teamwork skills.
