  • Supervise and direct Physical Therapist Assistants (PTAs) and rehabilitation aides.
  • Monitor resident progress and modify treatment plans to achieve optimal outcomes.
  • Complete accurate, timely documentation including evaluations, progress notes, recertifications, and discharge summaries.
  • Ensure compliance with Medicare (Part A & B), PDPM guidelines, and state/federal regulations.
  • Participate in interdisciplinary team meetings, care planning, and discharge planning.
  • Educate residents, families, and caregivers on treatment plans, safety, and home exercise programs.
  • Maintain productivity standards while delivering quality patient care.
  • Ensure a safe and effective therapy environment.

Position Summary

The PT focuses on restoring function, improving mobility, relieving pain, and preventing or limiting permanent physical disabilities. This role ensures high-quality care delivery while maintaining compliance with regulatory and reimbursement guidelines.

Key Responsibilities
  • Perform comprehensive evaluations of residents’ physical conditions, functional limitations, and rehabilitation needs
  • Develop and implement individualized treatment plans based on clinical findings and physician orders
  • Provide therapeutic interventions including exercise, manual therapy, gait training, balance training, and modalities
  • Supervise and direct Physical Therapist Assistants (PTAs) and rehabilitation aides
