Job summary

Alpha Home Health and Hospice in Illinois seeks a compassionate Physical Therapist to evaluate, plan, and implement individualized treatment to restore function, improve mobility, and maximize independence.

You will deliver skilled therapy, monitor progress, educate patients and caregivers, document care, and collaborate with physicians and the care team in a HIPAA-compliant, patient-centered environment.

Qualifications

• Graduate of an accredited Physical Therapy program with emphasis on patient assessment and treatment planning.
• Strong communication with patients, families, and care teams; ability to document clearly and accurately.
• Ability to think critically when evaluating progress and adjusting plans.

Job description

Key Responsibilities

Qualifications & Skills

• Graduate of an accredited Physical Therapy program (DPT preferred).
• Current or eligible Illinois Physical Therapist license.
• CPR/BLS certification or ability to obtain.
• One to two years of clinical experience preferred; new graduates encouraged to apply.
• Strong assessment, communication, and critical thinking skills.
• Ability to work effectively within a team and deliver compassionate, patient-centered care.
