Job summary

Fairlawn Health and Rehabilitation of Cascadia – Gresham, OR is seeking a Physical Therapist to join our growing therapy team in a well‑equipped 82‑bed facility. We offer guaranteed full‑time 40 hours per week with flexible scheduling, competitive pay ($52-$60/hour), and a comprehensive benefits package (medical, dental, vision, PTO, life insurance, disability, 401(k)).

Education: graduate from an accredited PT program; Licenses: current Oregon PT license and CPR certification; Experience: six

Qualifications

• Graduate from an accredited Physical Therapist program.
• Current Oregon PT license required; CPR certification.
• Six months of experience in a long-term care environment preferred.

Responsibilities

• Establish and administers a treatment plan with goals based on patient capacity.
• Assess patient needs and modify care to meet therapy goals.
• Collaborate with disciplines to plan and evaluate team goals for each patient.
• Set realistic and achievable goals for patients/residents.
• Document patient progress per state/federal regulations.
• Educate patients and families in therapy methods.
• Oversee screening to optimize functionality and skills.
• Communicate patient status to families and other professionals.

What You’ll Do:

A physical therapist provides services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of residents/patients suffering from injuries or disease.

• Establishes and administers a treatment plan with specific therapy goals determined according to the patient's capacity and tolerance under the direction of the physician.
• Assesses patient needs; plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
• Sets realistic and achievable goals for patients/residents.
• Documents and records the patients’/residents’ condition and progress per state and federal regulations.
• Educates patients and families in an appropriate therapy method.
• Optimizes the functionality and skills of patients by providing, directing and/or overseeing the screening of all residents.
• Communicates patient status and needs to the patient, family, and other involved professionals.
• Ensures punctuality and regular attendance for assigned shifts.
